Family Support

 

Our Family Support Services provide a continuum of resources that help caregivers reach higher levels of competency, success, and emotional well-being.

 

Programs include monthly caregiver trainings, bi-weekly support groups, counseling services, and intensive care management. As part of our mission to improve quality of life for ALL seniors in the community, many of our programs are complimentary or offered on a sliding scale and open to the public.

Care Managers

 

Care Managers at Concierge Care provide out-of-the box solutions, support, and resources for optimal aging. We strive to provide guidance, clarity, and momentum towards reaching care goals and maintaining your independence.

 

  • Evaluate your needs, including: crisis intervention, health, legal, housing, and community resources.
  • Develop a care plan that addresses concerns identified during the evaluation
  • Provide regular home visits to assess and supervise our home health aides
  • Attend care conferences with various members of the patient’s medical team
  • Arrange medical appointments
  • Participate in family meetings to review care options and advocate for the patient
  • Provide resources and referrals for additional services and support
  • Arrange additional therapies, including psychotherapy and physical and/or occupational therapies.
  • Serve as a “proxy” for out-of-town family members

Caregiver Counseling

 

Counseling services offer you support and the opportunity to share feelings in a safe environment. Counseling may help identify your needs and provide you with the best resources, education, and tools to cope effectively with the challenges of caregiving. All sessions are facilitated by a Master’s level Professional Clinical Counselor at an affordable fee. Topics include:

 

  • Adjusting to the Diagnosis
  • Crisis Management
  • Caregiver Skills-Building
  • Short-Term Problem Solving

Caregiving Essentials

 

This complimentary three-hour workshop, taught by David Hart Ph.D., is open to the entire family and empowers caregivers with the knowledge and coping skills needed to effectively manage the day-to-day challenges of caring for a loved one with memory impairment, Alzheimer’s disease, or a related dementia. In this workshop attendees will:

 

  • Receive an overview of dementia and Alzheimer’s disease, including the continuum of changes and stages of dementia and Alzheimer’s disease
  • Discuss effective strategies to communicate with persons in the early to late stages.
  • Learn adaptable techniques to help manage challenging behaviors
  • Process the emotional impact of caregiving and how to cope with challenging behaviors
  • Develop a plan to reduce stress and take care of yourself
