Our Memory Care Team, with David Hart, Ph.D., takes pride in our capacity to meet the needs of patients at every stage of the disease process, whether it’s Alzheimer’s disease or a related dementia.

We are committed to improving the quality of life for all affected clients and their families and care partners. We achieve our mission through the implementation of proven strategies shown to produce results for some of the more difficult challenges.

We offer specialized, empirically supported memory care for our patients. Our memory care programs are supervised by a registered clinician with 15 years of experience working with cognitively impaired patients.

We provide families with concrete results:


  • Our Memory Care Team (MCT) provides ongoing training for caregivers developed at Hoag Hospital to manage some of the more difficult behaviors frequently exhibited by patients with dementia.
  • Strategies include evidence based tools to communicate effectively with people with dementia, manage aggressive behaviours dues to confusion, and out-of-the-box approaches to stimulate patients cognitively, socially, physically, and spiritually.
  • Caregivers will receive ongoing supervision and education on specific patient needs related to the changing stages of dementia.
  • MCT will work with families to develop individually tailored Memory Care Plans that address the myriad of cognitive, behavioral, and functional needs present at varying stages of the disease process. The goal is to provide a higher quality of life for the patient and family.
  • Family members have access to several support services and educational opportunities and are assigned a primary Care Manager to supervise caregivers and oversee the implementation of the Memory Care Plan.
  • The Care Manager will provide support to the family or address any challenges that may arise and will visit with the family, caregiver, and patient frequently during the first 3 months of service. Hart is also available for family consultations and one-on-one counseling.
  • Our Memory Care Team is happy to provide resources and referrals to any community member in need of more affordable solutions.
